AmizaCare collects information so support requests can be reviewed safely and clearly. We handle personal information with care and reference the Australian Privacy Principles generally.

What information we collect

Website forms and referrals may collect names, contact details, suburb, plan type, NDIS number if provided, requested supports, goals, risks, communication preferences, preferred times and consent details.

Why we collect it

We use this information to respond to enquiries, check service fit, understand participant goals, plan safe support, prepare service agreements and communicate with approved people.

Consent and sharing

Information may be shared with participant-approved people such as nominees, family members, plan managers, support coordinators or workers involved in delivering support.

When sharing may be required

Information may be shared with authorities where legally required, including where safety, incident, safeguarding or reporting obligations apply.

Privacy concerns

If you have a privacy concern, want to correct information, or want to discuss who AmizaCare can speak with, contact us using the support email.

  • Tell us if consent or approved contact people have changed.
  • Do not send urgent medical or emergency matters by website form. Call 000 in an emergency.
